To begin the application process, please click the "Create an Account" tab at the top of this page to create an application account. From there, you will be instructed to create a login, password, and verify your email via a message to your email account.

You have the flexibility to log in and out of your application account without losing your data as it will autosave. There will be a green check mark for each section once you have completed all required fields.

3. Review and Submit & Pay Application Fee

Once all required fields of the application are complete, you must select "Review & Submit." You will then be instructed to pay the $30 application fee.

4. Request/Submit Required Documents

Please be sure that all required documents (if applicable) are provided to the admissions office:

OSLS Admissions Policy

Students applying for entrance into Preschool* must meet the following requirements prior to being reviewed for admissions:

1. All students applying to preschool must be the age of the coordinating grade on or before September 1st.
2. An Online Application must be completed with attached documents listed below:

a. A valid Certificate of Birth must be provided.
b. Florida School Entry Health Form (DH 3040): Must be issued within one year of enrollment.
c. Florida Certificate of Immunization (DH 680): Must be active (not expired).

3. The Online Application fee of $30 must be paid.
4. A Campus Tour, Enrollment Interview, and Student “Meet & Greet” will be required.

a. Parents/Guardians may schedule a campus tour either before or after application is submitted.
b. For preschoolers, 20 min. “Meet & Greet” will be required and may be scheduled during Parents’/Guardians’ Campus Tour.
c. The Enrollment Interview will be completed immediately following the Campus Tour.

*The term “Preschool” encompasses the two-year-old, three-year-old, and four-year-old Pre-Kindergarten (including VPK) programs.

Students applying for entrance into Kindergarten - 8th Grade must meet the following requirements prior to being reviewed for admissions:

5. All students applying for admissions must be the age of the coordinating grade
on or before September 1st.
6. An Online Application must be completed with attached documents listed below:

a. A valid Certificate of Birth must be provided.
b. Florida School Entry Health Form (DH 3040): Must be issued within one year of enrollment.
c. Florida Certificate of Immunization (DH 680): Must be active (not expired) and have appropriate immunization.
i. Kindergarten: Two Varicella Vaccinations
ii. 7th Grade: Tdap Vaccination

7. The Online Application fee of $30 must be paid.
8. A Campus Tour, Enrollment Interview, and Student Academic Assessment will be required.

a. Parents/Guardians may schedule a campus tour either before or after application is submitted.
b. The Enrollment Interview will be completed immediately following the Campus Tour.
c. Students must sit for an Academic Assessment and Classroom Shadow Day.
i. Shadow Days are offered on Tuesdays or Thursdays, half days.
d. All previous report cards, MAP test results, and letters of recommendation must be submitted during assessment.

Our Savior Lutheran School accepts new students for the current school year until the end of the First Quarter (late October), as long as there is space and that student meets all requirements for admissions.
Enrollment for the next school year will begin in January for current students and February for new students.
